Mesenchymal stem cells hold promise for regenerative medicine

摘要: Regenerative medicine is an emerging interdisciplinary field of research that uses several technological approaches including stem cells to repair tissues. Mesenchymal stem cells (MSCs), a type of adult stem cell, have generated a great amount of interest over the past decade in this field. Numerous studies have explored the role of MSCs in tissue repair and modulation of allogeneic immune responses. The mechanisms through which MSCs exert their therapeutic potential rely on some key properties of the cells as follows: the capacity to differentiate into osteoblasts, chondrocytes, adipocytes, cardiomyocytes, hepatocytes, endothelial, and neuronal cells; the ability to secrete multiple bioactive molecules capable of stimulating the recovery of injured cells and inhibiting inflammation; the lack of immunogenicity; and the ability to perform immunomodulatory functions. In the present review, we focus on these three aspects upon which the therapeutic effects of MSCs are mainly based. Furthermore, some pathological conditions under which the application of MSCs should be done with caution are also mentioned.

Therapeutic potential of stem cell in liver regeneration

摘要:

Liver transplantation is the only life-saving procedure for patients with end-stage liver disease. However, its potential benefits are hampered by many disadvantages, such as the relative shortage of donors, operative risks, and high costs. These issues have prompted the search for new alternative therapies for irreversible liver disease. Stem cell therapy, with the ability for self-renewal and potential for multilineage differentiation, is a promising alternative approach. Several studies have demonstrated that transplantation of hepatic stem/progenitor cells or hepatocyte-like cells derived from multipotent stem cells leads to donor cell-mediated repopulation of the liver and improved survival rates in experimental models of liver disease. However, a registered clinical application based on stem cell technology will take at least an additional 5 to 10 years because of some limitations; e.g. the lack of suitable cell sources and risk of teratoma formation. This review summarizes the general understanding of the therapeutic potentials of stem cells in liver disease, including the sources, mechanisms, and delivery methods of hepatic stem cells in liver regeneration, and discusses some challenges for their therapeutic application.

Microorganism-derived biological macromolecules for tissue engineering

摘要: According to literature, certain microorganism productions mediate biological effects. However, their beneficial characteristics remain unclear. Nowadays, scientists concentrate on obtaining natural materials from live creatures as new sources to produce innovative smart biomaterials for increasing tissue reconstruction in tissue engineering and regenerative medicine. The present review aims to introduce microorganism-derived biological macromolecules, such as pullulan, alginate, dextran, curdlan, and hyaluronic acid, and their available sources for tissue engineering. Growing evidence indicates that these materials can be used as biological material in scaffolds to enhance regeneration in damaged tissues and contribute to cosmetic and dermatological applications. These natural-based materials are attractive in pharmaceutical, regenerative medicine, and biomedical applications. This study provides a detailed overview of natural-based biomaterials, their chemical and physical properties, and new directions for future research and therapeutic applications.

我国细胞技术类再生医学创新型技术产业发展战略研究

近十年来国内外再生医学创新型医疗技术产业显示出了巨大的潜力和发展前景。本文通过查阅、整理和分析美国、欧盟成员国、日本、澳大利亚等典型发达国家在细胞技术类再生医学创新型医疗技术产业的现状和相关的政策法规、管理机制及其面临的问题,研究了我国在该技术产业的发展现状和政府的监管政策及管理机制,分析和总结了该产业的发展趋势、特点和存在的问题,提出以大力发展我国细胞技术类再生医学创新型医疗技术产业为原则,以突破产业发展的瓶颈为出发点的系列专家建议和意见。

Platelet-rich plasma: combinational treatment modalities for musculoskeletal conditions

Current research on common musculoskeletal problems, including osteoarticular conditions, tendinopathies, and muscle injuries, focuses on regenerative translational medicine. Platelet-rich plasma therapies have emerged as a potential approach to enhance tissue repair and regeneration. Platelet-rich plasma application aims to provide supraphysiological concentrations of platelets and optionally leukocytes at injured/pathological tissues mimicking the initial stages of healing. However, the efficacy of platelet-rich plasma is controversial in chronic diseases because patients’ outcomes show partial improvements. Platelet-rich plasma can be customized to specific conditions by selecting the most appropriate formulation and timing for application or by combining platelet-rich plasma with synergistic or complementary treatments. To achieve this goal, researchers should identify and enhance the main mechanisms of healing. In this review, the interactions between platelet-rich plasma and healing mechanisms were addressed and research opportunities for customized treatment modalities were outlined. The development of combinational platelet-rich plasma treatments that can be used safely and effectively to manipulate healing mechanisms would be valuable and would provide insights into the processes involved in physiological healing and pathological failure.

Chemical transdifferentiation: closer to regenerative medicine

Cell transdifferentiation, which directly switches one type of differentiated cells into another cell type, is more advantageous than cell reprogramming to generate pluripotent cells and differentiate them into functional cells. This process is crucial in regenerative medicine. However, the cell-converting strategies, which mainly depend on the virus-mediated expression of exogenous genes, have clinical safety concerns. Small molecules with compelling advantages are a potential alternative in manipulating cell fate conversion. In this review, we briefly retrospect the nature of cell transdifferentiation and summarize the current developments in the research of small molecules in promoting cell conversion. Particularly, we focus on the complete chemical compound-induced cell transdifferentiation, which is closer to the clinical translation in cell therapy. Despite these achievements, the mechanisms underpinning chemical transdifferentiation remain largely unknown. More importantly, identifying drugs that induce resident cell conversion in vivo to repair damaged tissue remains to be the end-goal in current regenerative medicine.

Evaluation of regenerative braking based on single-pedal control for electric vehicles

摘要: More than 25% of vehicle kinetic energy can be recycled under urban driving cycles. A single-pedal control strategy for regenerative braking is proposed to further enhance energy efficiency. Acceleration and deceleration are controlled by a single pedal, which alleviates driving intensity and prompts energy recovery. Regenerative braking is theoretically analyzed based on the construction of the single-pedal system, vehicle braking dynamics, and energy conservation law. The single-pedal control strategy is developed by considering daily driving conditions, and a single-pedal simulation model is established. Typical driving cycles are simulated to verify the effectiveness of the single-pedal control strategy. A dynamometer test is conducted to confirm the validity of the simulation model. Results show that using the single-pedal control strategy for electric vehicles can effectively improve the energy recovery rate and extend the driving range under the premise of ensuring safety while braking. The study lays a technical foundation for the optimization of regenerative braking systems and development of single-pedal control systems, which are conducive to the promotion and popularization of electric vehicles.

Regenerative braking control strategy in mild hybrid electric vehicles equipped with automatic manual

摘要: The actual regenerative braking force of an integrated starter/generator (ISG), which is varied with desired braking deceleration and vehicle speed, is calculated based on an analysis of the required deceleration, maximum braking force of ISG, engine braking force and state of charge (SOC) of battery. Braking force distribution strategies are presented according to the actual regenerative braking force of ISG. To recover the vehicle s kinetic energy maximally, braking shift rules for a mild hybrid electric vehicle (HEV) equipped with automatic manual transmission (AMT) are brought forward and effects of transmission ratios are considered. A test-bed is built up and regenerative braking tests are carried out. The results show that power recovered by the braking shift rules is more than that recovered by the normal braking control rules.

Individualized medicine, health medicine, and constitutional theory in Chinese medicine

摘要:

The patterns of modern science and changes in the medical model can result in the transformation of the current state of individualized and health medicines into being the primary trend in medical development. Chinese and Western medical systems are dissimilar in terms of value orientations, thinking style, and research directions because of their different historical and cultural backgrounds. Individualized treatment in modern medicine is mainly established based on individual genome information and the differences in mononucleotide polymorphi-sms. However, such treatment method is expensive, creates an uncertain genetic marker, and leads to different result interpretations, among other problems. The Chinese constitutional theory developed in the 1970s expresses the principle behind Chinese health medicine and individual treatment and provides the corresponding methods. The Chinese constitutional theory divides the constitution of the Chinese population into nine categories based on established classification criteria. It promotes the study of the relationship of each constitution to diseases and Chinese medicine preparation toward adjusting the constitution and preventing diseases. The theory also provides methods and tools for individualized treatment. Constitution identification shows the direction and provides the core technology for the evaluation of the health status. By combining the developments in modern biotechnology, new diagnostic techniques and treatment models of constitution-differentiation, disease-differentiation, and syndrome-differentiation can be established for the development of individualized Chinese medicine treatment and health medicine for the international medical community.

摘要: The gradual development of Chinese medicine is based on constant accumulation and summary of experience in clinical practice, but without the benefit of undergoing the experimental medicine stage. Although Chinese medicine has formed a systematic and unique theory system through thousands of years, with the development of evidence-based medicine, the bondage of the research methods of experience medicine to Chinese medicine is appearing. The rapid transition and transformation from experience medicine to evidence-based medicine have become important content in the development of Chinese medicine. According to the features of Chinese medicine, we propose the research idea of “taking two ways simultaneously,” which is the study both in the ideal condition and in the real world. Analyzing and constructing the theoretical basis and methodology of clinical research in the real world, and building the stage for research technique is key to the effective clinical research of Chinese medicine. Only by gradually maturing and completing the clinical research methods of the real world could we realize “taking two ways simultaneously” and complementing each other, continuously produce scientific and reliable evidence of Chinese medicine, as well as transform and develop Chinese medicine from experience medicine to evidence-based medicine.

Clinical research of traditional Chinese medicine in big data era

With the advent of big data era, our thinking, technology and methodology are being transformed. Data-intensive scientific discovery based on big data, named “The Fourth Paradigm,” has become a new paradigm of scientific research. Along with the development and application of the Internet information technology in the field of healthcare, individual health records, clinical data of diagnosis and treatment, and genomic data have been accumulated dramatically, which generates big data in medical field for clinical research and assessment. With the support of big data, the defects and weakness may be overcome in the methodology of the conventional clinical evaluation based on sampling. Our research target shifts from the “causality inference” to “correlativity analysis.” This not only facilitates the evaluation of individualized treatment, disease prediction, prevention and prognosis, but also is suitable for the practice of preventive healthcare and symptom pattern differentiation for treatment in terms of traditional Chinese medicine (TCM), and for the post-marketing evaluation of Chinese patent medicines. To conduct clinical studies involved in big data in TCM domain, top level design is needed and should be performed orderly. The fundamental construction and innovation studies should be strengthened in the sections of data platform creation, data analysis technology and big-data professionals fostering and training.

以疫病流行及其原型分析为线索,联系早期治法(如热熨、火灸、砭刺等)探讨了古代以腺鼠疫为代表的疫病流行与医学起源间的内在联系;以医源于疫的视野分析我国医史病史研究在出土文物、古文字和医学文献等方面丰富的信息资源与优势;分析解读疾病名实的古今变迁、比较中西医学及其在认识论与方法论上的差异,为审视我国传统医学在疫病防治方面的丰富经验与延至今日的独到成就与意义,疾病史研究成果与推广,实现古为今用,将为当今疫病防治的中西互补、中西医结合与比较提供宝贵的历史经验与借鉴。
